Admin doesn't always get informed about new users etc. via e-mail

Description
Usually, every time a new user has been created or a user has changed his password the admin gets informed about those events via e-mail. Unfortunately, that's not always the case. It seems to be some kind of hit-or-miss situation. How can that be?
