Sometimes the upload doesn't work (documents do not appear)

Description

When we add documents (after uploading, filling in the meta information and pressing the "send"-button) the documents do not appear in the overview. They seem to be gone, we can't find them anywhere. This happens several times (up to 50%) by all users and all kind of files (e.g. ppt, doc).

It's very frustrating, so please solve this problem very soon.

What browaser are you using?

If you reload the folder, does the document appear or not?

We are using Firefox 8 and IE8. Reload doesn't help.
